How It Works

Under federal law, your doctors and hospitals must give you a copy of your records when you ask. Democura turns the asking into one tap, and keeps everything that comes back in one place, in order, in plain language.
Sign up once, then check in anywhere with a scan. You decide what each office can see and for how long, and you can end it whenever you like. The front desk gets what it needs. Nothing more leaves your hands.
Switch plans. Move across the country. See a specialist for the first time. Your whole history goes with you, and the new office starts with the full picture instead of a blank form.
The Democura App
These parts are built and in testing with made-up records. The screens are the real app, running a test practice; the phone is a drawing.
Fill in your forms from a private link before you arrive, or on the iPad at the front desk. Either way, they land in the same place, and you only tell your story once.
Open it from a private link and your date of birth. Your record, your next visit and whether your forms are done, and the plan your doctor signed, with steps to check off.
Show a check-in code at the front desk, or send a link. You choose what each office can see, and the link stops on its own.
An AI assistant you can talk to about your plan, by chat or by voice. It is off unless you turn it on, it knows only your own record, and anything your care team should know goes to them.
Your records belong to you. Download them, print them, or give another doctor a link that stops working when you say.
